LETTER OF AUTHORIZATION

According to FCC Regulations you must fill out a Letter of Authorization (LOA) to establish new phone service or to move your phone service to LocalTel Communications. By filling out this electronic LOA you will give LocalTel Communications authority to act as your agent in establishing new phone service or in moving your existing phone service to LocalTel.

LocalTel will then become your Local Exchange Telephone Carrier. This form will also authorize LocalTel to switch your long distance plan to LocalTel Long Distance if you choose the appropriate option below.

Please Note: All information submitted on this form is strictly confidential and will not be shared with any person or company outside your current Local Exchange Telephone Carrier, LocalTel Communications or NWInternet without your prior authorization.

By filling out and submitting this form, you are submitting an "electronic signature", giving LocalTel Communications authorization to act as your agent and to become your Local Exchange Telephone Carrier.
